Please join Drs. Linda Mitchell, Nidhi Shah and Tina Wexler to learn more about what matters most when communicating medical decisions through Advance Directives.  An Advance Directive is a way to document and empower one’s wishes, values, and goals for medical treatment in the event of lost capacity. The Drs. will also address Palliative Care, a specialized medical discipline that leverages a holistic approach to treating patients and families suffering through serious illnesses.

Tina Wexler, MD, is an attending physician with the Montefiore Medical Center Palliative Care Program and an Assistant Professor of Family and Social Medicine at the Albert Einstein College of Medicine.

Linda Mitchell, MD, is currently the lead physician in Palliative Care at Yale New Haven Health - Greenwich Hospital and a former Assistant Professor of Family and Social Medicine at Albert Einstein College of Medicine, Montefiore Medical Center. She is board certified in Family Medicine, Geriatrics, and Hospice and Palliative Medicine.

Nidhi Shah, MD, is an accomplished physician with a diverse background in Hospital Medicine, Hospice and Palliative Medicine, and Geriatric Medicine. She is currently serving as the Associate Director of Hospitalist Services with the Department of Medicine at Greenwich Hospital. She is board certified in Internal Medicine, Geriatrics, and Hospice and Palliative Medicine.
